1) Diver is lost - Reinstall the correct driver for the card.

2) Wrong driver - Remove the failed driver and software, reinstall the correct driver and software

3) Card is not in the slot properly - Make sure the card completely plugged into the PCI slot.

4) Old version of card may not be compatiable with the new version software - downgrade the new version to an older version
